Gateway to Care Collaborative

To apply to become a member or affiliate of the Gateway to Care  Collaborative, call Gateway to Care at 713.783.4616 for an application. When submitting Application, please include a narrative describing your services.

As per the By-Laws of the Gateway to Care Collaborative, a Member of the Gateway to Care Collaborative has voting rights and is obligated to attend at least 60% of the Collaborative meetings and to serve on committee(s). Gateway to Care's Core Values are available.

An Affiliate does not have voting rights, but can serve on committee(s).
